The obesity epidemic and the beneficial effects of very low-calorie diets

It is well known that the prevalence of obesity has reached epidemic proportions, and that the disease is associated with various health risks, morbidity, and even mortality. Investigators are currently in search for safe and effective treatment options to reduce the risk of the associated health hazards, such as increased risk of diabetes mellitus, hypertension, cardiopulmonary and thromboembolic disease, hyperlipidemia, and sudden cardiac death. In fact, the National Institutes of Health Consensus Development Conference on the Health Implications of Obesity confirmed the need for effective treatments for obesity and stressed the urgency of reducing the mortality rates associated with the disease. The panel concluded that individuals who are as little as 20% over their ideal weight are at an increased risk of morbidity and mortality, and that one-fourth of all adult Americans are at least this much overweight. It is quite clear that obesity is a major health problem and that the most important step in preventing obesity, or reversing obesity for an obese individual, is losing the excess weight. Unfortunately, a very small proportion of those individuals who need to lose the excess weight are successful. In 1959, a literature review indicated that less than 5% of obese individuals who need to lose greater than 18kg were successful. Since then several methods of weight reduction have been developed, which include fasting, hypocaloric balanced diets, behavior modification, pharmacotherapy, surgery, and a combination of these methods. Unfortunately, most methods have proven to produce only transient weight loss and are often accompanied by dangerous, adverse side-effects.

Nearly 50 years of extensive research, however, has resulted in some progress in the treatment of obesity. The use of very-low-calorie-diets (VLCDs) (roughly 500-800 calories per day) has significantly contributed to this progress, despite the controversies about their safety and effectiveness in long-term weight loss. The existing literature, however, seems to support the belief that, when under the direct supervision of a physician, VLCDs provide more health benefits than risks and appear to rapidly reduce the incidence and severity of the various complications of obesity (including diabetes mellitus, hypertension, and various mechanical problems such as sleep apnea and joint problems). For example, in a population of obese individuals with hypertension (46%), two-thirds became normotensive and most of the remaining participants experienced reduced blood pressure, serum lipid levels, and triglyceride levels, as well as increased carbohydrate tolerance and improved pulmonary function. Other proposed benefits of VLCD-induced weight loss include increased exercise tolerance, improved self-esteem and sense of wellbeing, and alleviation of joint and low back pain commonly associated with carrying excess weight. While the benefits of VLCDs are clinically significant and improve the overall health of obese individuals, the above-mentioned improvements are not permanent if the positive stimulus is removed. Say hello to Wello, goodbye to exercise excuses

1. From your backgrounds, it's obvious that you're both active and health-conscious - but how'd the idea for Wello come about?

We were in business school at Stanford and had a really hard time finding time to fit in a workout. Since making it to the gym or a fitness class of some sort seemed to be out of the question for our hectic schedules, we decided to try doing the Insanity workout with a bunch of friends. The only problem was we were all at really different fitness levels and had different needs. One of the girls started developing really big biceps (that she didn't want!), another girl was struggling to keep up, and we just felt like it was off - that there had to be something better! So, we asked a trainer to Skype with us. He said he'd never done it before but was willing to give it a try and it kicked our butts! From that moment, we realized we were on to something. Training at home could be better more effective, more personal and more fun if you had a real live trainer interacting with you as you worked out. And that's how Wello was born.

2. What benefits does remote training offer that a gym can't?

Remote training allows you the flexibility to work out when you want, where you want. You no longer have to circle a parking lot or sit in traffic to get to the gym. You don't have to rush home to make a class. You can do it on your time. It's also great if you're new to working out, travel frequently or have young kids at home. You can really fit it into your day so working out has never been easier. Also, Wello is much cheaper than hiring a trainer in a gym!

3. How have you addressed any concerns about the concept of working long-distance with a trainer?

We simply ask that people give it a try. It works. We have so many happy Wello customers who will tell you that having your trainer watching you via our two-way video makes you accountable and work harder than you ever will with any workout DVD. Plus, the trainer is so much more focused on you and what you are doing. You can also see them, so they can't look off in the distance or start high fiving other trainers at the gym their central focus is you.

4. Who are finding to be the most avid Wello users so far?

Our most avid Wello users are busy professionals, moms with young kids and fitness enthusiasts who love the freedom to work out when they want, where they want and with a trainer of their choice. We also have a ton of users who are using Wello to accomplish big goals, such as lose 25 pounds or finish a marathon.

5. Any surprise discoveries or trends you're noticing as people use the site?

Great question! We were actually quite surprised at how many people are using Wello to try workouts that they've never done before like Pilates or yoga it's a great way to discover new workouts and try something new from the privacy of your home.

6. Where's the oddest place that you or a client has "Wello'd?"

We have a client who Wello's in his garage and another on her roof deck. We have also had some beautiful pictures of people Wello'ing on vacation.

7. What's next on the horizon for Wello, and what's your long-term vision for the company?

We want to become a health and wellness platform that not only provides workouts over live, two-way video, but also a range of wellness services, such as nutrition counseling, physical therapy, weight loss coaching, cooking lessons and more.

Why you should never go on a diet again

It is estimated that 45 million Americans go on diets each year. The number of dieters is highest after the holidays. Yet only 5% keep the weight off. You may have heard the diets don't work, but in desperation to lose weight, you try every new diet on the market.

A wise reporter once said, "You can put less bread on your meat or less meat on your bread, but the only way you're going to lose weight is to burn more calories than you take in".

Weight loss is a simple concept with complex challenges. Society, stress, family, cravings, fatigue, lack of self control...these are all things that get in the way of losing weight and maintaining weight loss. The only way you're going to have life long results is to make life long lifestyle changes you can stick with.

Most "diets" are unrealistic, short-term and some, down right dangerous. You should never go on a diet, you should have a diet. The best way to judge if an eating program is healthy is that is should incorporate all of the major food groups. When you're living on a sustainable, healthy diet, very little is entirely off limits.

When looking for a weight loss program, look for one that teaches you how to live, not just sells you products. Look for support, tips, advice from real people, and sustainable lifestyle changes. If it's not something you could do forever, chances are you shouldn't be doing it. Find exercise you enjoy doing and food you enjoy eating. Meet a 'Digest Diet' success story

Liz Vaccariello, editor-in-chief and chief content officer of "Reader's Digest," wanted to use the most up-to-date science on weight loss and healthy eating to create her latest book. "The Digest Diet" (Reader's Digest, $24.99) takes that information and lays out a 21-day eating plan that doesn't neglect any major food groups and offers up loads of superfood-based recipes to make losing weight safe and effective.

Vaccariello, who had a big hit a few years ago with the Flat Belly Diet collection of books that she coauthored, borrows a bit from that series with this balanced eating plan that also features monounsaturated fats, known as MUFAs.   3. Days 15 through 21 you "Finish Strong" with a plan that balances carbs, protein and fats.

There are lots of menu suggestions and some great recipes, most with big color photos. Some of the meals include Oatmeal Breakfast Cakes, Lemony Lentil & Chard Soup, Chile-Rubbed Pork Tenderloin, and even Fudgy Mocha Brownies.

Vaccariello's assistant, Adrienne Farr, was one of the test subjects for the diet before its release. We spoke with her in August at the BlogHer conference in New York, where Vaccariello and Reader's Digest previewed the book. Farr is featured in the book with an 18-pound loss, but when we spoke with her, she was down nearly 50 pounds and couldn't wait to tell the world about her success. Biggest Loser season 11 makeover week highlights (Photos)

Just got in all the fairytale makeover photos from last night's Biggest Loser makeover week show.

Different from seasons past, this makeover week was more like getting the final five contestants ready for "Biggest Loser Prom," and the prom band was OneRepublic -- which we didn't see much of.

Past makeovers have had more of a fashion show feel, with contestants wearing either fashionable everyday clothes or cocktail party wear. But last night's show was full-on Oscar night stuff.

Was it actually too over the top? With the uber-sparkly gowns and elbow-length gloves it was almost hard to envision the contestants as slimmer and fitter people.
